CARE COORDINATOR - SIX MONTH CONTRACT

Coolamon Shire Council is seeking a compassionate, organised and motivated Care Coordinator to join our team on a 6-month full-time contract.

Allawah Community Care has been a trusted provider of care and support services for the elderly in the Coolamon Shire for over 25 years. In this role, you will support clients to maintain their independence and wellbeing by coordinating high-quality, person-centred services.

We offer entry-level support services under the Commonwealth Home Support Program (CHSP) for those requiring basic assistance, as well as a government-funded Support at Home Programs (SAH) for those requiring more complex care.

Our CHSP services include domestic assistance, transportation, social support, home maintenance, home modifications, meals and respite, while our SAH services cover a wider range of support services, including personal care and medication assistance.

The Care Coordinator is responsible for supporting and coordinating the delivery of care services by Allawah Community Care to enable clients to live independently and ensuring service delivery complies with government funding programs and policies.

To be successful in this position you must be able to work with minimal supervision, have excellent people and communication skills and be able to prioritise tasks.
